Job Description

Job Summary  
A Case Manager provides service facilitation for assigned individuals in Medicaid waiver programs (CILA, Community Day Services and Supported Employment). This is a crucial role in supporting individuals with intellectual and developmental disabilities to navigate various aspects of their lives. The primary focus is to enhance the quality of life for individuals with IDD by coordinating and advocating for the necessary services and resources.

Pay: $21 -$25 per hour
Job Type: Full-Time
 
Essential Duties and Responsibilities  
  1. Conduct initial and ongoing assessments to identify and understand unique needs, strengths, and challenges of each individual (such as ICAP, SLOF, RISK, MEDICAL, SNAPSHOT.)   
  2. Collaborate with individuals, their families, and other professionals to develop holistic implementation strategies.  
  3. Advocate for the rights and interests of individuals, ensuring that they have access to appropriate services, education, and community resources (consents.)  
  4. Coordinate and monitor the delivery of services to individuals, ensuring that their needs are met.  
  5. Maintain accurate and up-to-date records of each individual’s progress and services received.  
  6. Be prepared to manage and respond to crises or emergencies, providing immediate support and coordination of necessary resources.  
  7. Work with other professionals and agencies to address any legal or rights-related issues affecting individuals.  
  8. Work closely with other professionals, including social workers, healthcare providers, educators, and community organizations, to provide a comprehensive support system for individuals.  
  9. Provide aid and training in daily living skills to help individuals achieve greater independence.  
  10. Develop and implement programs that focus on enhancing the individuals’ abilities to live in their communities.  
  11. Facilitate community involvement and integration for individuals, promoting socialization and inclusion in various community activities (natural supports.)   
  12. Maintain open communication with individuals, their families, and support staff to address concerns, provide updates, and ensure a collaborative approach to care.  
  13. Train direct care staff, providing guidance and support in implementing individualized service plans.  
  14. Ensure compliance with relevant regulations, policies, and procedures governing the care and support of individuals with developmental disabilities.  
  15. Provide training to staff, individuals, and families on relevant topics, including disability awareness, communication strategies, and crisis management.  
  16. Complete and maintain accurate documentation, including progress notes, incident reports, and other required paperwork.  
  17. Prepare and submit reports as required by regulatory agencies.  
  18. Provide coverage in Medicaid waiver programs as needed. Attend trainings to meet annual requirements, including obtaining 12 CEUs annual to maintain employment status as a Case Manager.
  19. Model and support Lifelong Access Core Values in all areas and situations across the organization.
  20. Knowledge of and adherence to agency policies and procedures.  
  21. Employee may be asked to perform other duties as required by the employer’s business needs.  

 
Qualifications  

  • Bachelor’s degree in Social Work, Nursing, Psychology, Public Health, or a related field.
  • Minimum one year of experience supporting people with disabilities.
  • Some supervisory or management experience preferred but not required.
  • Must be QIDP certified, or eligible to obtain certification within the first three months of assuming role.
  • Demonstrated knowledge of Microsoft Office applications, including Excel, Word, and Outlook , Paycor.
  • Must be able to communicate effectively, both verbally and in writing .
  • Must be able to obtain CPR/First Aid certification.
  • Valid Illinois driver’s license with safe driving record .
  • Current proof of automobile insurance .


Working Conditions
The working environment for this job is a changing one that adjusts to the needs of the individuals supported. At times, the environment will be fast paced with interruptions, distractions, and deadlines. While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.

Occasionally required to stoop, bend, crouch, push, pull and reach. Specific vision abilities required by this job include clear close and distance vision. Must be able to occasionally lift and/or move up to 50 pounds.
