HOSPICE ADMISSIONS REGISTERED NURSE

Dallas–Fort Worth, Texas

A hospice admission is more than a clinical assessment—it is often the first moment a patient and family begin to feel supported, understood, and confident that they will not face the hospice journey alone.

Our confidential client is a well-established, locally operated North Texas hospice provider known for compassionate, high-quality patient care and an outstanding team-oriented culture. The organization is seeking an experienced Hospice Admissions Registered Nurse who can combine sound clinical judgment with the empathy, communication, and professionalism needed to guide patients and families through one of life’s most difficult transitions.

POSITION OVERVIEW

The Hospice Admissions Registered Nurse serves as the initial clinical point of contact for prospective hospice patients and their families throughout the Dallas–Fort Worth market.

The Admissions RN will evaluate patients for hospice eligibility, clearly explain hospice services and benefits, complete the admission process, and initiate an individualized plan of care that creates a seamless transition into hospice services.

The position works closely with clinical leadership, physicians, referral sources, and the interdisciplinary hospice team to ensure every patient receives a timely, compassionate, and well-coordinated admission experience.

This position reports to the Clinical Manager, with additional leadership support from the Chief Operating Officer.

SERVICE TERRITORY

The primary service area includes:

  • Collin County

  • Dallas County

  • Denton County

  • Tarrant County

  • Cooke County

Regular local travel throughout the assigned Dallas–Fort Worth territory is required.

KEY RESPONSIBILITIES

  • Evaluate and assess prospective patients to determine clinical eligibility for hospice services.

  • Review diagnoses, medical history, current symptoms, functional status, and supporting clinical documentation.

  • Communicate effectively with physicians, referral sources, patients, families, and caregivers throughout the admission process.

  • Explain hospice philosophy, services, benefits, levels of care, and patient and family expectations with clarity and compassion.

  • Guide patients and families through emotionally difficult conversations in a calm, respectful, and supportive manner.

  • Complete all required admission assessments, consents, documentation, and supporting clinical records accurately and promptly.

  • Initiate individualized plans of care based on the patient’s clinical condition, symptoms, goals, and immediate needs.

  • Coordinate medications, equipment, supplies, and other services necessary for a safe and timely transition into hospice care.

  • Collaborate with the Clinical Manager and interdisciplinary team to ensure seamless onboarding of every new patient.

  • Communicate significant clinical findings and changes in condition promptly to the appropriate team members.

  • Maintain accurate and timely documentation in accordance with company, Medicare, and regulatory requirements.

  • Manage daily admission activity independently while maintaining consistent communication with clinical leadership.

  • Participate in limited on-call responsibilities as needed.

  • Assist with additional clinical or administrative functions as assigned.

  • Strong clinical assessment and critical-thinking skills.

  • Excellent verbal, written, and interpersonal communication abilities.

  • Strong documentation, organization, and time-management skills.

  • The ability to work effectively as part of an interdisciplinary team.

  • The independence and judgment necessary to manage a daily schedule with minimal supervision.

  • Flexibility and adaptability when admission timing or patient needs change unexpectedly.

  • A dependable, professional, and patient-centered approach.

  • Proficiency with electronic medical record systems.

  • Experience with Consolo is preferred.

REQUIRED QUALIFICATIONS

  • Current, unrestricted Texas Registered Nurse license.

  • Minimum of one year of hospice nursing experience.

  • Working knowledge of hospice eligibility requirements and the hospice admission process.

  • Valid driver’s license.

  • Reliable personal transportation.

  • Current automobile insurance.

  • Ability to travel throughout the assigned Dallas–Fort Worth service territory.

  • Ability to participate in limited on-call responsibilities.

SCHEDULE

  • Full-time

  • Schedule flexibility based on patient admission needs

  • Limited on-call responsibilities as needed

  • Local travel throughout Collin, Dallas, Denton, Tarrant, and Cooke counties

COMPENSATION

  • Base salary of $85,000–$90,000 annually, based on experience and qualifications

  • Mileage reimbursement at $0.65 per mile

  • Cell phone reimbursement of $60 per month
